Why lapses are missed
Nothing fails. A confirmation is a signature at an interval — if it isn't made, no machine stops and nobody is notified. It's discovered when a certificate is pulled for an audit, long after the window closed, when the only route left is requalification.
What it doesn't check
The dates are half the question. The qualified ranges — thickness, diameter, position, material group — decide whether this welder covers this joint at all. That's a calculation the certificate carries, not a date.
Doing this properly
A calculator does one certificate at a time. What removes the problem is the system knowing every welder's status and refusing to assign one to a joint they no longer cover.
