Why lapses are missed
Nothing fails. A confirmation is a signature at an interval: if it is missed, nobody is told. It surfaces at an audit, when the only route left is requalification.
What it doesn't check
The dates are half the question. The qualified ranges (thickness, diameter, position, material group) decide whether this welder covers this joint at all.
Doing this properly
A calculator checks one certificate at a time. The problem goes away when the system knows every welder's status and refuses a joint they no longer cover.
