Nonconformance and corrective action
A nonconformance raised in welding, found by NDT and dispositioned by engineering usually lives in three systems and agrees with itself in none of them.
How it works

Raise it where it was found
From the weld, the examination, the inspection point or the incoming material — with the origin recorded, not retyped.

Disposition it
Use as is, repair, rework, reject, or concession. With who decided, on what basis, and whether the client had to agree.

Close the loop
A repair produces new work with its own examination. The nonconformance is not closed because the paperwork was filed.

Escalate when it repeats
A pattern across jobs becomes a corrective action, with a cause and a verification that it worked.

Internal by default
A nonconformance register that is visible to the client by default does not get used. People stop raising things, or they raise them somewhere else, and the register becomes a record of what was safe to admit rather than a record of what happened.
So the default is internal, and making something visible to the client is a deliberate act by someone with the authority to make it. That is not concealment: the dossier still carries what the contract requires it to carry. It is the difference between a system people tell the truth to and a system they manage.
