NCR — Nonconformance Report
The record of something that did not meet its requirement, what was decided about it, and by whom.
The disposition is the substance: use as is, repair, rework, reject, or concession. Each has consequences — a repair generates new work with its own examination, a concession requires the client’s agreement.
Internal by default
A register visible to the client by default stops being used. People raise fewer things, or raise them elsewhere, and it becomes a record of what was safe to admit. Making an entry client-visible should be a deliberate act by someone with the authority, not a side effect of how permissions were configured.
